Date of Birth * You will need this to track your order.

Country of Birth *

The country where you were born, as printed in the “Place of birth” field of your passport. This is not your citizenship or the country you live in now.

Application / File Number * Shown on your IRCC letter / AOR (e.g. E…, W…, V…, S…, B…).

UCI Number * 8 or 10 digits. Dashes are added automatically.
